Use the enrollment option that matches your situation. New families will request an account first; returning families can use the Student Records Portal.

First-time families to MPS

For families who have never attended Mannford Public Schools or have never had a child enrolled at MPS.

• An active email and mobile phone are required to verify your account.
• Go to the Sylogist portal and click Request an Account.
• Follow the prompts and complete the requested information.
• After verification, look for an activation email from SylogistED.
• The activation link is valid for seven days.
• Once complete, you need to open the Student Records Portal to complete the enrollment: https://oked.sylogist.com/mannford

Current MPS family adding a new student

For families who already have a student at Mannford Public Schools and need to enroll an additional child.

• Log in to the Sylogist portal with your username and password.
• Use the Student Records Portal to complete the new enrollment.
• If you have forgotten your password, email [email protected] with the name on your account. You will receive an email from Sylogist with a link that will allow you to reset your password.

Returning family updates

For families with students already at MPS who are moving to a new site or updating annual demographic information.

• Update phone numbers and email addresses annually.
• Review demographics and emergency contacts.
• Use the Sylogist portal to keep student records current.

Make sure you are using the correct Mannford Sylogist link. Older Wengage links may no longer work.

Login troubleshooting

Try these fixes if you are having trouble with the gradebook, lunch account, or Student Records Portal.

Check the app. If using the Wengage by SylogistEd app, make sure it is updated. You may also delete and reinstall it.

Use the correct bookmark. The correct link is oked.sylogist.com/mannford. The old wengage.com/mannford link no longer works.

Clear browser data. You may need to clear cookies and cache for all time, then make sure your browser is up to date.

Reset your password. Email [email protected] with the name on your account. You will receive an email from Sylogist with a reset link.

Immunizations and health records

For school enrollment, a parent or guardian should provide either current, up-to-date immunization records or a completed and signed exemption form.

To schedule back-to-school immunizations, call the Creek County Health Department at 918-224-5531 or 918-367-3341. If your child received immunizations somewhere other than the County Health Department, please provide a copy of the immunization record before the appointment to help reduce wait time.

Birth certificates and shot records

Birth certificates may be ordered online or by calling 877-817-7364. Shot records may be available through the Oklahoma State Immunization Information System public portal.

To use the OSIIS portal, you will need the child's name, date of birth, and the contact phone or email connected to the child's shot records.

Required enrollment documents

Have these items ready when completing enrollment. Some items apply only when relevant to the student or family situation.

Legal Guardianship or Custodial AgreementRequired if parents are divorced or the student is living with someone other than a parent.

Previous School InformationInclude last report card, withdrawal sheet, transcript, phone, fax, website, or address when available.

Special Health or Medical Condition NoticeProvide if applicable.

Permission to Administer Medicine FormProvide if applicable.

Free and Reduced Lunch FormProvide if applicable. This must be renewed annually.

Indian Education Form 506Provide if applicable. JOM requires a Tribal Membership Card, letter, or CDIB.

Home Language SurveyComplete the Home Language Survey as part of enrollment paperwork.
