How to Enroll in Mannford School


HOW TO ENROLL AT MANNFORD PUBLIC SCHOOLS

First Time Families to MPS/Never Attended MPS/Never had a Child at Mannford Public Schools:
  • An active email and mobile phone will be required to verify your account.
  • Go to https://oked.sylogist.com/mannford 
  • Click on the Request an Account..
  • Follow the prompts and complete the information.
  • After your information has been verified, you will receive an email from SylogistED with an activation link.
  • Link is valid for seven days.
  • Once complete you need to open the Student Records Portal to complete the enrollment: https://oked.sylogist.com/mannford  

Families with students currently at MPS but also with a new student to enroll:
If you know your username and password, login at https://oked.sylogist.com/mannford  and use the Student Records Portal to complete the enrollment.

If you have forgotten your password:
  • Email [email protected]s with your name on your account.
  • You will receive an email from SylogistEd with a link which will allow you to reset your password.

If you have forgotten your username,  your username will be sent via email so you can complete the process.

Families with students at MPS who are moving to a new site or just needing to update demographic information, please go to  https://oked.sylogist.com/mannford  to update your phone numbers, email, demographics, and emergency contacts annually.

Potential fixes for log-in issues with the Gradebook/Lunch Account program - Sylogist (formerly wengage)

1. If using the wengage by SylogistEd app on your mobile device, be sure it is up to date with the current release. You may also delete it and re-download it. Be sure the app is titled wengage by SylogistEd.
2. If using a browser on your mobile device or any browser from a computer/laptop (Safari, Chrome, Edge etc.) be sure that the correct link is bookmarked.  3. You may need to clear the cookies/cache from your browser. Because computers and mobile devices store these cookies until they are deleted, an "all time" clear may be necessary. 
4. Be sure the browser is up to date with the current release.
5. Users can reset their own passwords by clicking the "FORGOT PASSWORD" link on the SylogistEd login page. ​


For school enrollment, a parent or guardian shall provide one of the following: (1) current, up-to-date immunization records; or (2) a completed and signed exemption form. To schedule an appointment for your child’s back-to-school immunizations, please call the Creek County Health Department at 918-224-5531 or 918-367-3341. If your child has received immunizations somewhere other than the County Health Department, please provide a copy of their immunization record prior to the day of the appointment. This will assist in reducing your wait time.

Birth certificates can be ordered online or by calling 877-817-7364. Requests from a parent named on the record are typically issued in two business days and arrive in the mail typically within a week.
If a parent or guardian needs to get a child’s shot records those can be found online through the Oklahoma Immunization Information System (OSIIS) portal.
  • To obtain shot records through the portal you must have the child’s name, date of birth, and the contact phone or email on the child’s shot records.
  • If the records are not in OSIIS, you can contact the OSIIS Help Desk by emailing [email protected] or you can reach out to the provider who administered the shots and ask them to update OSIIS.

6.  Legal Guardianship or Custodial Agreement (required if parents are divorced      or student is living with someone other than parent)

7.  Previous school information (last report card, withdraw sheet, high school        transcript, phone, fax, website, address)        
8.  Special Education Records if applicable

9.  Notification of Special Health/Medical Condition if applicable

10.  Permission to Administer Medicine Form if applicable

11.  Free and Reduced Lunch Form if applicable (must be renewed annually) 

12.  Indian Education Form 506 if applicable 
      (JOM requires Tribal Membership Card/Letter or CDIB7.
